2. 2024 Event-Vendor-Application-City (download pdf below)
3. Proof of Liability Insurance with the Apache Junction Area Chamber of Commerce listed as additional insured
4. Pinal County Health Permit
5. City of Apache Junction Business License Number
6. TPT (Transaction Privilege Tax) Number
7. Menu and Photo(s) of Food Truck Setup
